Background & Objective: Diabetes has emerged as one of the world’s biggest health problem that faces the health care providers because of its various complications and high prevalence percentage.

Diabetes is traditionally known as a silent disease, exhibiting no symptoms until progression to target organ damage.

Patients' adherence with total medical plan is considered the corner stone to control diabetes and its complications.

Many studies indicated that diabetic patients presented a low level of compliance in medical recommendations including blood glucose monitoring, medications, food regimen, physical activity and foot-care .

This study was conducted to assess Syrian diabetic patients' compliance with medical recommendations.

Materials & Methods: A sample of 300 diabetic patients (163 male&137 females) aged 20-70 years interviewed in Lattakia diabetic center- ministry of health.

Results: The results revealed that 81.7% of patients had one or more than one type of diabetes complications.

82 % of them were not committed with self -monitoring of their blood glucose level.

80 % of them did not monitor their weight, 63% of them did not follow their planned nutritional regimen and 42.5 % of them did not provide appropriate and preventive care of their feet.

Conclusion: Taking medications was the most frequently that diabetic patients were adhered to; while there is declining in patients' adherence to physical activity, nutritional regimen and foot-care; consequently studied patients are vulnerable to acute or chronic complications.
