Background and Aim: ideal root canal obturation which provides three dimensional seal of the root canal system and creates a favorable environment for periapical healing, represents the basic and essential purpose of the root canal treatment.

With the growing interest in the biological benefits offered by bioactive bioceramic materials in root canal obturation, especially its effectiveness in stimulating healing of periapical tissues, the aim of this study was to investigate the effect of bioactive bioceramic materials on the healing of periapical lesions.

Materials and Methods: The sample of In vivo study consisted of 40 human permanent (upper or lower) mature necrotic molars with periapical lesions.

Root canals were prepared with ESX rotary system, the sample was randomly divided into 2 equal groups (n= 20 molar); group 1: root canals were obturated with [GP + Endosequence BC Sealer], and group 2: root canals were obturated with [Endosequence BC Points + Endosequence BC Sealer] with (MSCT) technique in both of them.

Then the clinical cases were followed up at 4 different time intervals (1 month, 3 months, 6 months, and 12 months.

Data were subjected to Mann-Whitney U statistical test to study the significance of differences in the frequency of root canal treatment success between the two groups, with (P-value ≤ 0.05) as the level of significance.

Results: Both of the studied groups showed success in the endodontic treatment, without statistically significant differences.

Conclusions: Within the limitations of this in vivo study, it could be concluded that root canal filling sealer Endosequence BC Sealer has contributed to the overall healing of periapical lesions, especially when shared with Endosequence BC Points.
