Background : Inflammation plays an important role in the pathogenesis and progression of many forms of heart failure.

In the last decade, the concept that WAT represents an important determinant of a chronic inflammatory state, has gained more relevance at the same time as providing a potential link between obesity and cardiovascular disease through the induction of endothelial dysfunction by a chronic, low-level inflammation.

Recently, studies have shown that elevated CRP levels correlate with New York Heart Association (NYHA) functional class.

In these diseases, increased serum levels of adiponectin are correlated with increased serum levels of CRP, and leptin.

Aim of the study: Do the hs-CRP concentrations in patients with heart failure differ from those in healthy ? Does any correlation exists between hs- CRP and adipocytokines (leptin, adiponectin) ? Methods : Our study included 67 pateints with congestive heart failure divided into four groups according to (NYHA) Functional Class, and 21controls subjects.

Serum hs-CRP, adiponectin, and leptin levels were measured by enzyme linked immunosorbent assay (ELISA).

Results: Serum hs-CRP levels were statistically significant increased (P = 0.0002) in patients with CHF compared with controls (1.7 ± 1.2 µg / ml), but that elevation has no statistically significant according to the NYHA class, NYHA II : 4.8 ± 3.7 µg/ml, NYHA III: 5.9±3 µg/ml, NYHA IV : 7.9 ± 2.6 µg / ml.

controls/ II (P = 0.01), II/ III (P = 0.2), III/ IV (P = 0.02).

Conclusion : These findings suggest that an elevated level of hs-CRP is an independent predictor of prognosis in CHF and can provide additional prognostic information for the risk stratification and treatment in patients with CHF.
