Endothelin-1 is a 21 amino acid peptide, and is the most potent vasoconstrictor, generated by vascular endothelial cells in response to a variety of chemical and mechanical signals.

Endothelin-1 contributes to the pathophysiology of conditions associated with sustained vasoconstriction, such as hypertension and heart failure.

Serum levels of endothelin-1 were measured by Enzyme Immunoassy in 137 subjects (27 normal subjects, 110 heart failure patients, who were clinically classified according to the New York Heart Association (NYHA) classification to NYHA II (31 patients), NYHA III (49 patients), and NYHA IV (30 patients).

Serum levels of Endothelin-1 were found to be significantly higher in heart failure patients compared with normal subjects, P value was <0.05, also Serum levels of Endothelin-1 were significantly associated with the severity of heart failure.

Our study elucidates that the serum levels of endothelin-1 are elevated in patients with heart failure and are associated with the progression of the disease, so routine serum endothelin-1 determination provides important prognostic information in heart failure
