Background & Objective: Onychomycosis is a common fungal infection, mainly caused by superficial dermatophytes, the clinical features of onychomycosis is variable, - onycholysis - fragility- color modifications- and subungual hyperkeratosis, diagnosis is based on direct examination using KOH, and fungal culture on Sabouraud medium.

The treatment of onychomycosis is based on systemic antifungal and topical filmogenic solutions, the treatment of choice of onychomycosis is based on oral Terbinafine in continuous treatment ( 250 mg/day for 6 weeks in finger onychomycosis, and 3 months in toes onychomycosis), which is an expensive medication and cost high for the patient and the medical economy.

many trials were experienced in order to find other therapeutic choices less expensive, Itraconazol was suggested as pulse therapy ( 200- 400 mg/day for one week / month, repeated 4 times).

purpose of the study: to find an economic, not expensive and effective way of treatment by the drug most effective in Onychomycosis which is Terbinafine.

Methods & Materials: in this study included 34 patients with onychomycosis, the patients were randomly divided to 2 groups: - group 1: included 18 patients received oral Itraconazol 200 mg twice a day for one week.

- group 2: included 16 patients with onychomycosis, received oral Terbinafine 250 mg twice a day for one week a month..

each medication was administrated for 4 monthly pulses.

the control was done monthly, during the trial, and for 6 months after the end of the treatment., control was based on clinical improvement and the negativity of direct examination by KOH.

Conclusion: As a result we found that both pulse therapies of Itraconazol, and Terbinafine were almost equal in efficacy and side effects.
