Background and Objective: Alendronate is used today for inhibiting bone resorption in several diseases in medical field.

In this study, the effects of alendronate on the distraction osteogenesis were evaluated.

Methods and Materials: Twelve patients (3 males, 9 females)(mean age 20 years) with transverse maxillary deficiency were included in this study.

All patients received buccal corticotomy with palatal osteotomies and received maxillary expansion.

The post-operative activation rate was 0,8mm/day.

Alendronate was applied topically in the anterior palatal osteotomy region (between two central incisors).

All patients were evaluated radiographically using cone-beam computerized tomography at the pre-operative (T0), 3 months after expansion (T1) and 6 months after expansion.

The bone density was evaluated in both anterior region (alendronatewas applied) and posterior region (alendronate wasn't applied).

Result: There were statistically significant differences (P< 0.05) between posterior and anterior region.

The bone density change ratio was 32.42%in anterior region at 6 months after expansion.

Conclusion: Local alendronate accelerates bone healing in the osteotomy site when used with distraction osteogenesis.
