Background and Objective: inferior alveolar nerve block (IANB) is the primary method used to achieve mandibular analgesia.

There are many reasons for failure The purpose of this study is the evaluation of reasons of failure.

Methods and Materials: this study included 1700 patients who needed to extracted one of their teeth in the mandible 900 males were ranging between (15-60years) with mean age of 42 years, and 800 females were ranging between (22-55 years)with mean age of 36 years ,Who had been treated by students in the fifth year of the University of Damascus faculty of dentistry in the clinics of tooth extraction ,between 20112013 Each student who failed to obtain a successful anesthesia by inferior alveolar nerve using one cartilage of 1,8 ml of lidocain2% with epinephrine 1:80,000 is required to tell the researcher.

The researcher questioned the patient and conduct clinical examination and make sure that the anesthesia has failed ,he performs a Panoramic radiograph and in case of there is no reason related to the patient, whether topical or anatomical ,the researcher inject the patient accurately and in case of success or failure of anesthesia the researcher fill the special form.

Statistical analysis spss were used with p<0,05 Results: The failure rate was 10,6% the most common cause of mandibular block failure is due to wrong Technique ,Anatomical variation, infection, volume factors, fear of patient, expiry date of local anesthetic, insufficient waiting period after anesthesia, alcohol .

Conclusion: The student must master the inferior alveolar nerve block (IANB) because the wrong Technique is the most important reasons for failure and should also pay attention to other factors.
