Background &Objective There is an obvious tendency in hand surgery reconstruction towards regional and free flaps.

On the other hand, abdominal flaps are decreasing in their importance due to their disadvantages and pitfalls.

Although these flaps are almost forgotten in most of the units around the world, they are still an important choice for reconstruction in posttraumatic hand – and forearm – in the developing countries and that is due to their simple technique which make them an easy solution for almost all types of tissue defects in the hand; the lack of surgical experiences in microsurgery plays a big major in determining the choice of flap.

It is to find out the importance of abdominal flaps in hand posttraumatic injuries detailing our experience in AL Mouassat University Hospital.

Methods & Material: Seremty- eight cases with defects in the upper limb due to burns or trauma were examined retrospectively between 2009-2014.

All defects were treated with flaps and results were analyzed.

Results: Fifty-seven defects were covered with abdominal flaps.

Inguinal flaps were used to cover five defects, regional flaps were used in 14 patients, two cases were covered with free flap.

Conclusion: Abdominal flaps were the first choice for hand and plastic surgeons in the upper limb posttraumatic and burns reconstruction in our country it gives good outcomes despite its higher coast due to long hospitalization and sick leave
