ABSTRACT: Background: Adipose tissue secretes many adipocytokines, such as TNF-? and IL-6, that were reported to have a role in the insulin resistant state associated with obesity or type 2 DM.
IL-1? was also reported to be secreted by adipose tissue, however its role in pathogenesis of obesity associated insulin resistance is not yet established.
Objective: The aim of this study was to study the relation between IL-1? levels, and both insulin resistance (assessed by HOMA-IR), and ?-cell function (assessed by HOMA-?) in obese type 2 diabetics, as well as obese subjects with either impaired or normal glucose tolerance.
Study Design: The study was conducted on 50 subjects divided as follows: Group 1 (diabetics): it included 20 obese type 2 diabetic patients, with a mean age of 45.9±5.8 years, and a mean BMI of 38.58±5.35.
Group 2 (obese-IGT): it included 10 obese subjects with impaired glucose tolerance (IGT), their mean age was 44.3±11.97 years, and mean BMI of 37.85±7.45.
Group 3 (obese-NGT): it included 10 obese subjects with normal glucose tolerance (NGT), their mean age was 44.6±8.55 years, and a mean BMI of 38.9±5.97.
Group 4 (control): it included 10 normal weight healthy subjects; their mean age was 39.7±9.62 years, and a mean BMI of 22.02±0.90.
The following were done to all the subjects in the study; anthropometric measures including waist circumference, BMI, laboratory investigations including; fasting blood glucose, 2 hour postprandial, or 2 hour post glucose load blood glucose, triglycerides, total cholesterol, fasting insulin (FI), and IL-1?.
HOMA-IR and HOMA-B were calculated.
Results: FI and HOMA-IR were significantly higher in the diabetic group (FI=41.0±14.1uIU/ml-HOMA-IR=20.9±10.09),in the obese-IGT(FI=45.4±9.16uIU/ml- HOMA-IR=12.49±2.37), and in the obese-NGT(FI=50.8±19.29uIU/ml-HOMA-
