Pyoderma gangrenosum (PG) is a rare neutrophilic dermatosis.
his clinical and biological picture is nonspecific which often leads to a diagnosis delay.
we report the case of pyoderma gangrenosum, whose clinical picture consisted of recurrent skin abscess despite drainage and antibiotic therapy with subsequent ulcerative lesions associated with asymptomatic pulmonary localization.
secondarily, there was onset of arthritis of the knee leading to arthrotomy and discontinuation of corticosteroid therapy.
outcome is made towards the appearance of new lesions about the wound of the arthrotomy justifying the reintroduction of the steroids with a good clinical outcome.
we must therefore think of pyoderma gangrenosum in front of abscesses or recurrent skin ulcers.
its prognosis is improved by a diagnosis as well as an early corticotherapy.
الملخص FRE
Pyoderma gangrenosum (PG) est une dermatose neutrophilique rare.
son tableau clinique et biologique est non spécifique ce qui entraine souvent un retard diagnostique.
nous rapportons l’observation d’un cas de pyoderma gangrenosum dont le tableau clinique se résumait à des abcès cutanés récidivants malgré le drainage et l’antibiothérapie avec apparition ultérieure de lésions ulcéreuses associée à une localisation pulmonaire asymptomatique.
secondairement, il y a eu apparition d’une arthrite du genou ayant conduit à une arthrotomie et à l’arrêt de la corticothérapie.
l’évolution s’est faite vers l’apparition de nouvelles lésions en regard de la plaie de l’arthrotomie justifiant la réintroduction de la corticothérapie avec une bonne réponse clinique.
il faut donc penser au pyoderma gangrenosum devant des abcès ou des ulcères cutanés récidivants.
son pronostic est amélioré par un diagnostic ainsi qu’une corticothérapie précoce.
