الموضوع : يعد الدور الذي يلعبه نقص الحديد في التسبب في اضطراب فرط الحركة و قلة الانتباه أحد العوامل المطروحة للبحث في هذا المجال.
و ذلك نظرا للدور الذي يلعبه الحديد في إنتاج مادتي الدوبامين و النورأدرينالين.
و تهدف الدراسة الراهنة إلى تقييم حالة الحديد في دماء عينة من الأطفال المصريين المصابين باضطراب فرط الحركة و قلة الانتباه.
طرائق البحث : تم اختيار ثلاثين طفلا مصابا باضطراب فرط الحركة و قلة الانتباه (بين عمر السادسة و الاثني عشر عاما) بالإضافة إلى خمسة عشر طفلا صحيحا كمجموعة ضابطة.
و قد تم تقييم الأطفال عن طريق المقياس العالمي المصغر النفسي العصبي للأطفال (ميني كيد) - النسخة العربية, و مقياس كونر المعدل للوالدين.
كما تم تقييم حالة الحديد في دماء الأطفال عن طريق تحليل صورة الدم الكاملة, و تقدير مستوى الحديد, و القدرة الإجمالية على استيعاب الحديد, و مستوى مادة الفيرتين بالدم.
النتائج : سجل الأطفال المصابون باضطراب فرط الحركة و قلة الانتباه مستويات أقل - بنسبة دالة إحصائيا -من الأطفال الأصحاء فيما يختص بمستوى مادتي الهيموجلوبين و الفيرتين بالدم.
كما تم تسجيل تناسب عكسي قوي الدلالة بين مستوى مادة الفيرتين و المقياس الكلي لفرط الحركة و قلة الانتباه بالإضافة إلى المقياس المعرفي الجزئي المعبر عن قلة الانتباه, بينما لم يتم تسجيل تناسب مشابه مع مقياس فرط الحركة كما لم يتم تسجيل أي فروق دالة بين المجموعتين في بقية المؤشرات المعملية التي تم قياسها.
خلاصة البحث : هناك مؤشرات تدل على أن نقص الحديد يلعب دورا هاما في التسبب في اضطراب فرط الحركة و قلة الانتباه سواء في وجود علامات الانيميا أم في غيابها.
و هناك حاجة إلى المزيد من البحث - خاصة في الدول النامية - لتقييم المدى الذي يمكن أن يلعبه العلاج التعويضي للحديد في علاج اضطراب فرط الحركة و قلة الانتباه.
Abstract EN
Objective : Iron deficiency was suggested to play a role in the pathophysiology of attention deficit hyperactivity Disorder (ADHD) due to the role of iron in the production of dopamine and noradrenaline.
The aim of the present Study was to assess iron status in a sample of Egyptian children with ADHD.
Methods : Thirty children with ADHD (6-12 Years old) and 15 age and gender matched controls were recruited.
Subjects were evaluated using the Mini International Neuropsychiatric Interview for kids (mini-kid) and Conner's Parent Rating Scale - Revised (CPRS-R).
Laboratory measures Were also carried out to investigate iron status in all subjects including: complete blood count, serum iron, total iron binding Capacity, and serum ferritin.
Results : Children with ADHD showed significantly lower levels of hemoglobin and serum Ferritin as compared to control groups.
A strong negative correlation was found between serum ferritin and ADHD total Score and cognitive sub score, but not with hyperactivity sub score.
No significant differences were found between the two Groups as regards other hematological data.
Conclusions : Iron deficiency, with or without anemia, might play a role in Pathogenesis of ADHD.
Further work is needed to confirm the clinical utility of iron supplementation in the treatment of ADHD, especially in developing countries.
