Students’ evaluation of faculty member is one of the effective styles and influential in improving and upgrading the performance of teaching and guidance.

It also shows the weaknesses and strengths, which will benefit students and the faculty member together.

The success of the evaluation process depends on the tools that used in it ; this study aims to design an assessment form to evaluate faculty members by students from at college of Arts and Science at Rabigh and Kamel, king Abdul Aziz university.

The objective of the study is to prepare a proposal model assessment, which included five themes : personal characteristics, preparation and implementation of the lecture, human relations scientific mastery and assessment.

The sample study consisted of 90 faculty members and analyzed the data were for the percentages, averages and standard deviations.

Results showed that all study sample accepted the consensus and phrases contained in the five themes .The study proved the importance of personal characteristics as honesty, appropriate appearance and emotional balance of the faculty member.

It also proved the importance of establishing a human contact with students as well as the instructor's competence and mastering the academic content.

The study also stressed the importance of using a multiplicity of teaching methodologies and classroom management as well as the instructor's capability of handling lectures, and it recommended the need of using the five consensus and phrases as tools in the evaluation of faculty member.
