Back ground: Bentall procedure is considered a typical procedure for replacement of aortic route consisting of the replacement of the aortic valve and reimplantaion of the coronary arteries which associated with enlargement in ascending aorta for more than 45-50 mm.
However this operation is a high risk one with mortality rate about 12%.
The leading cause of death is bleeding so modified techniques have been appeared to decrease the bleeding rate.
Case series: Here we demonstrated four different cases of surgical diseases of the ascending aorta which managed with modified Pentall-Takriti operation.
Discussion: The preserving of insufficient aortic valve and fixing of the new valve with to the aortic annulus support the fixing stiches and do not leave any dead spaces between stiches (the leading cause of the surgical bleeding).
At the same time the biologic bleeding could be controlled with transfusion of blood derivities .
Result: The final results of these cases was excellent with mortality rate 0% and without any important complications.
The discharge from hospital was after one week of operation.
