يعتبر التهاب العظم و النخاع العظمي في العظام المطروحة كيانا سريريا نادرا تشخيصه غير شائع و صعب و يتطلب شكا كبيرا.
أبلغنا عن حالة التهاب عظم لوح الكتف من جهة الأخرم لدى طفل في يبلغ 14 سنة.
تم الاشتباه في التشخيص لوجود أوجاع عظمية حادة مصحوبة بحمى.
كان التصوير الطبي بالأشعة السينية عاديا.
أبرز التصوري بالصدى فوق الصوتي و التصوير المقطعي المحسوب للكتف وجود كتلة متجمعة ملامسة للأخرم.
تم علاجه بواسطة التنظير الجراحي و بالمضادات الحيوية المناسبة و تم رصد و عزل المكورات العنقودية الذهبية في العينات المأخوذة أثناء الجراحة.
كان بعد ذلك التطور جيدا في مدة 4 سنوات.
Abstract EN
Acute osteomyelitis of flat bones is a rare clinical entity.
Its diagnosis is uncommon, difficult, and requires a high suspicion.
We report a case of scapular osteomyelitis in a 14-year-old male infant involving the acromion.
The diagnosis was clinically suspected in front of acute bone pain with fever.
Standard radiography was normal.
Ultrasonography and CT-Scan of the shoulder have objectified a collection in contact with the acromion.
He was treated by an urgent surgical debridement through a posterior approach and adapted antibiotic therapy.
Staphylococcus aureus was isolated in preoperative specimens.
The evolution was good after 4 years of follow-up.

L’ostéomyélite aigüe des os plats est une entité clinique rare.
Son diagnostic est non commun, difficile et exige une haute suspicion.
On rapporte le cas d’une ostéomyélite scapulaire touchant l’acromion chez un garçon âgé de 14 ans.
Le diagnostic était suspecté cliniquement devant une douleur osseuse aigüe avec fièvre.
La radiographie standard était normale.
L’échographie et la tomodensitométrie de l’épaule ont objectivé une collection au contact de l’acromion.
Il a été traité par un débridement chirurgical en urgence avec une antibiothérapie adaptée.
Un Staphylocoque aureus a été isolé dans les prélèvements peropératoires.
L’évolution a été bonne avec un recul de 4 ans.
