Dissection de l’aorte chez une femme enceinte : a propos d’un cas

Aortic dissection is a rare and a serious condition in which a tear occurs in the inner layer of the body's main artery (aorta) We report the case of a 27-year-old woman, already diagnosed with Marfan's disease and operated 1 year ago for Bentall surgery.

She was pregnant at 7 weeks of amenorrhea.

A therapeutic termination of pregnancy was proposed after recent cardiologic consultation and exploration for transfixing chest pain: Transesophageal echocardiography showed type III aortic dissection and Computed tomography angiography confirmed the dissection of thoracic and abdominal aorta type B of the Stanford classification.

This dissection was extended to the aortic bifurcation.

The patient was informed of the high anesthetic risk.

The act took place without incident.

La dissection aortique est une affection rare et grave caractérisée par l'irruption de sang à l'intérieur de la paroi de l'aorte.

Nous rapportons le cas d’une femme âgée de 27 ans, porteuse de la maladie de Marfan, enceinte à 7 semaines d’aménorrhée, opérée il y a 1 an pour chirurgie Bentall qui a été prise en charge pour un arrêt thérapeutique de la grossesse.

Elle a consulté en cardiologie pour des douleurs thoraciques.

L’échocardiographie transoesophagienne a montré une dissection de l’aorte type III.

L’angiotomodensimétrie a été demandée et a confirmé la dissection de l’aorte thoracique et abdominale de type B de Stanford étendue jusqu’à la bifurcation aortique.

La patiente a été informée du haut risque anesthésique.

L’acte s’est déroulé sans incidents.
