Facteurs de risque d’hypocalcemie apres thyroïdectomie totale

Abstract EN

Objectives: to describe the incidence and assess the risk factors for hypocalcemia after total thyroidectomy.

Method: we conducted retrospective descriptive and analytic study of patients who have had a total thyroidectomy over a period of 6 years.

the factors studied were: age, sex, indication and duration of surgery, histological type, parathyroidectomy and the performance of a central lymph node dissection.

Results: hypocalcemia was transient in 31.2% and definitive in 5.9% of patients.

in univariate analysis, the presence of parathyroid in the surgical specimen is a predictive factor for transient hypocalcemia.

duration of surgery, central lymph node dissection, and malignancy were associated with transient and definitive hypocalcemia.

in multivariate analysis, the presence of parathyroid in the surgical specimen was the risk factor for transient hypocalcemia and no factor influenced the occurrence of definitive hypocalcemia.

Conclusion: involuntary parathyroidectomy is an independent risk factor for transient hypocalcemia.

Objectifs:décrire l'incidence et évaluer les facteurs de risque de l’hypocalcémie après thyroïdectomie totale.

Methodes:etude rétrospective descriptive et analytique portant sur les patients ayant eu une thyroïdectomie totale sur une période de 6 ans.

les facteurs étudiés ont été: l’âge, le sexe, les indications, la durée de la chirurgie, le type histologique, la parathyroïdectomie et la réalisation d’un évidement médiastinorécurrentiel.

Resultats: l’hypocalcémie a été transitoire chez 31,2% et définitive chez 5,9%des patients.

A l’analyse univariée, la présence de parathyroïde dans la pièce opératoire était un facteur prédictif d’hypocalcémie transitoire.la durée de la chirurgie, l’évidement médiastino-récurrentiel et la malignité étaient associés à l’hypocalcémie transitoire et définitive.

a l’analyse multivariée, la présence de parathyroïde dans la pièce opératoire était le facteur de risque d’hypocalcémie transitoire et aucun facteur n’a influencé la survenue d’hypocalcémie définitive.

Conclusion: la parathyroïdectomie involontaire est un facteur de risque indépendant d’hypocalcémie transitoire.
