هدف البحث : تهدف هذه الدراسة إلى استقصاء دور الإنتان بفيروس التهاب الكبد C في تطور النمط الثاني للداء السكري عبر تقييم المستويات المصلية للإنترفيرون غاما، العامل المنخر للورم ألفا و الإنترلوكين-10.
طرق البحث : شمل التقييم 36 من مرضى النمط الثاني للداء السكري في مركز أمراض الغدد الصم و السكري في بغداد.
تراوحت أعمار المرضى بين 15 و 70 سنة، لدى 21 منهم داء سكري من النمط الثاني فقط، و لدى 15 آخرين داء سكري مع إنتان بفيروس التهاب الكبد C.
كما شملت الدراسة 13 شخصا من الأصحاء دون أية أعراض أو علامات مرضية كمجموعة شاهد.
تم تقييم المستويات المصلية للإنترفيرون غاما، العامل المنخر للورم ألفا و الإنترلوكين-10 من خلال المقايسة المناعية الامتزازية المرتبطة بالأنزيمELISA.
النتائج : لوحظ وجود مستويات أعلى من الإنترفيرون غاما و العامل المنخر للورم ألفا عند مرضى الإنتان بفيروس التهاب الكبد C مع الداء السكري (43.87 بيكوغرام / مل و 68.1 بيكوغرام / مل على الترتيب) بالمقارنـة مع مرضى النمط الثاني للــــداء السكري دون وجود إنتان (19.75 بيكوغـرام / مل و 55.10 بيكوغرام / مل على الترتيب)، و مجموعة الشاهد (8.08 بيكوغرام / مل و 31.4 بيكوغرام / مل على الترتيب).
أظهر التحليل الإحصائي وجود فارق معنوي بين المجموعات الثلاث السابقة (p = 0.01) لوحظ أن متوسط القيم المصلية للإنترلوكين-10 كان أعلى و بشكل كبير عند مجموعة مرضى النمط الثاني للداء السكري المعزول بالمقارنة مع مجموعة مرضى النمط الثاني للسكري مع الإنتان بفيروس التهاب الكبد C و مجموعة الشاهد (28.7، 10.32 و 15.78 بيكوغرام / مل على الترتيب، قيمة p = 0.05).
الاستنتاجات : قد تلعب الزيادة في إنتاج السيتوكينات طليعـة الالتهابية (الإنترفيرون غامــــا و العامل المنخر للورم ألفا) و انخفاض مستويات الإنترلوكين-10 دورا هاما في إمراضية الإنتان بفيروس التهاب الكبد C و الذي يقود لتطور الداء السكري من النمط الثاني عبر زيادة المقاومة للأنسولين.
Abstract EN
Objective : The aim of this study is to investigate the role of HCV infection in the development of type diabetes through the assessment of IFN-γ, TNF-α and IL-10 serum levels.
Methods : Thirty six known type 2 diabetes patients attending the endocrine and diabetes center in Baghdad for check-up were underwent this study.
Their age ranged from 15-70 years.
Twenty one patients have type 2 diabetes only while the other fifteen were have diabetes with HCV infection.
Thirteen healthy individuals without any signs or symptoms of disease were also included in this study as healthy controls.
Serum levels of cytokines including IFN-γ, TNF-α and IL-10 were analyzed by ELISA immunoassay.
Results : Higher serum levels of IFN-γ and TNF-α were observed in the investigated HCV patients with diabetes (43.87 pg / ml, 68.1 pg / ml, respectively) compared to type 2 diabetes patients (19.75 and 55.10 pg / ml, respectively and controls (8.08 and 31.4 pg / ml respectively).
The statistical analysis revealed α significant difference between these three groups (p=0.01).
The mean serum levels of IL-10 were significantly elevated in type 2 diabetes group as compared to HCV patients with type diabetes and control groups (28.7, 10.32, 15.78 pg / ml 2 respectively, p=0.05).
Conclusions : The overproduction of proinflammatory cytokines (IFN-γ, TNF-α) and the low IL-10 level could play a crucial role in pathogenesis of HCV that leads to type 2 diabetes via increasing the insulin resistance.
