من الممكن توضيح التهاب العصب السابع في شلل بل باستخدام التصوير بمادة الجادولنيوم.
و قد قمنا بدراسة مجموعة مكونة من واحد و عشرين مريضا مصابون بشلل بل باستخدام التصوير بالرنين المغناطيسي بهذه المادة.
بينهم خمسة عشر مريضا يعانون من التهاب حاد بالعصب السابع و ستة مرضى يعانون من التهاب مزمن.
كان الهدف من هذه الدراسة هو تسجيل النتائج و توضيح أهميتها و تقييم دور التصوير بالرنين المغناطيسي في مرضى شلل بل.
و أوضحت النتائج أن معظم المرضى لديهم انتشار واسع في امتصاص مادة الجادولنيوم في العصب السابع عادة في الجزء الأخير من قناة الأذن الداخلية و أجزاء التية و الدكية.
أما الأجزاء الأخرى فكانت أقل امتصاصا.
و لم يلاحظ أي فرق في شكل الامتصاص بين المرضى المصابون بالالتهاب الحاد أو المزمن و قوة الامتصاص تحدث بدون النظر لشدة الإصابة بشلل العصب و التشخيص بالاختبارات الكهروفسيولوجية، و أن يختفي الامتصاص في الأشعة بعد الشفاء الإكلينيكي.
و تبين أن الأجزاء التي بها امتصاص لمادة الجادولنيوم هي الأجزاء المصابة بالشلل.
و لذلك نوصي باستخدام التصوير بالرنين المغناطيسي بمادة الجادولنيوم للأشخاص المصابين بشلل العصب السابع المتكرر و غير المألوف.
و مع التشخيص الإكلينيكي السليم يمكن التشخيص عن طريق استخدام التصوير بالرنين المغناطيسي التصوير بالرنين المغناطيسي بمادة الجادلنيوم بالتباين في حالات شلل بل و قد كان من قبل تشخيص هذه الحالات بلا استثناء و كذلك ليستثنى احتمال وجود أي أورام مصاحبة للعصب السابع.
Abstract EN
We studied a series of 21 Bell’s palsy patients with Gadolinium-enhanced magnetic resonance imaging (Gad.MRI), 15 in the subacute and 6 in the chronic stage.
The purpose of this study was to report our findings, discuss their significance, and evaluate the role of MRI in Bell’s palsy.
Most of our cases demonstrated diffuse facial nerve enhancement, usually in the distal internal auditory canal and labyrinthine/geniculate segments.
Other segments demonstrated enhancement less often.
No difference was noticed in the pattern of enhancement between the acute and chronic stages.
Enhancement occurred regardless of the severity of the paralysis (evaluated by electrophysiological tests) and radiological resolution appeared to lag behind clinical resolution.
The enhancement pattern in the Bell’s palsy group corroborates that the segments most often involved in Bell’s palsy are the segments that are most often enhanced with Gad.MRI.
Thus, section contrast enhanced MRI scans are recommended for individuals with recurrent, atypical presentation of facial paralysis.
In the proper clinical setting, contrast–enhanced MRI may permit a positive radiological diagnosis of Bell’s palsy, which has previously been a diagnosis of exclusion, and excludes the possibility of associated facial nerve neoplasms.
