في هذا البحث تمت دراسة و مقارنة مستوى الأجسام المضادة للخلايا المبطنة لجدران الأوعية الدموية و الجزيئات اللاصقة الذائبة بخلايا الأوعية الدموية-1 و الجزء اللاصق إي سيليكتين في مصل 30 مريضا بالرثيان المفصلي و مصابين بالتهاب الأعصاب الطرفية مع 30 مريضا بالرثيان المفصلي و غير مصابين بالتهاب الأعصاب الطرفية و مجموعة ضابطة مكونة من 25 متطوعين أصحاء.
و تم في هذا البحث أيضا دراسة علاقة الأجسام المضادة للخلايا المبطنة لجدران الأوعية الدموية و الجزيئات اللاصقة الذائبة مع النشاط المرضي.
وجد أن مستوى الأجسام المضادة للخلايا المبطنة لجدران الأوعية الدموية و الجزء اللاصق بخلايا الأوعية الدموية-1 و الجزء اللاصق إي سيليكتين قد زادت زيادة بدلالة إحصائية في مرضى الرثيان المفصلي المصابين بالتهاب الأعصاب الطرفية عن مستواهم في مرضى الرثيان المفصلي الغير مصابين بالتهاب الأعصاب الطرفية و المجموعة الضابطة من الأصحاء.
كما وجد أيضا أن هناك علاقة ذات دلالة إحصائية بين مستوى الأجسام المضادة للخلايا المبطنة لجدران الأوعية الدموية و الجزيئات اللاصقة الذائبة و النشاط المرضى في مرضى الرثيان المفصلي المصابين بالتهاب الأعصاب الطرفية.
و كنتيجة شاملة للبحث وجد أن معرفة مستوى الأجسام المضادة للخلايا المبطنة لجدران الأوعية الدموية و الجزيئات اللاصقة الذائبة يمكن استخدامها في متابعة مرضى الرثيان المفصلي أثناء العلاج و استخدامها أيضا في معرفة طرق العلاج.
Abstract EN
Objective : To examine whether levels of anti-endothelial cell antibodies (AECA), soluble vascular cell adhesion molecule-1 (sVCAM-1 and E-selectin were elevated in rheumatoid arthritis (RA) patients complicated with peripheral neuropathy (PN) as compared to RA patients without neuropathy and healthy controls.
Also, to correlate these criteria with the parameters of disease activity.
Methods : Levels of AECA, sVCAM-1 and E-selectin were determined with ELISA in 30 RA patients with PN, 30 RA patients without neuropathy and 25 healthy controls.
Results : RA patients with PN had significantly higher levels of AECA, sVCAM-1 and E-selectin than RA patients without neuropathy and healthy controls.
Those three markers significantly correlated with the clinical and laboratory parameters of disease activity.
Conclusion : Serum levels of AECA, sVCAM-1 and E-selectin proved to be higher in RA patients with PN than in RA patients without neuropathy.
This reflects increased endothelial cell activation in RA patients complicated with neuropathy.
