According to the ADA guidelines, Metformin and lifestyle modifications are the first line therapies of type 2 diabetes mellitus.

However, Metformin causes reduction of vitamin B12 and folic acid absorption.

Therefore, we studied the effect of Metformin on vitamin B12 and folic acid absorption in 99 type 2 Diabetic patients (Metformin group 42 patients and Metformin plus sulfonylurea group 57 patients), by measuring body mass index (BMI), vitamin B12, folic acid, calcium, albumin, iron and haemoglobin concentration, at baseline of our study and after 6 months (Study period), we also measured glycated haemoglobin (HbA1c), lipid profile and creatinine concentration.

The results demonstrated that treatment with Metformin accompanied with reduction of vitamin B12 absorption, concentration of folic acid, hemoglobin and glycated hemoglobin, but there was no mal absorption of calcium, iron and albumin.

And there was no positive effect on lipid Profile or body mass index (BMI).
