الهدف : يجري تشخيص داء الليشمانيات الحلدي (CL) في سورية وفقا للطرق التقليدية باستخدام الاستعراف المجهري و الزرع.
و هما الأكثر شيوعا بسبب نوعيتهما العالية.
إلا أنه من ناحبة أخرى، ما زالت عدة مساوئ تجعل تطبيقها محدودا.
و من بينما حساسيتها المنخفضة و فترة الاختبار.
و طرأ لأن الأبحاث العالمية أظهرت أن طريقة تضخيم دنا منشأ الحركة kDNA PCR عالية العباسية و النوعية، لذلك كان هدف بحثنا مقارنة حساسية الفحص المجهري و الزن kDNA PCR3 اعتمادا على عينات محلية.
المواد و الطرق : أخذت عينات من 70 مريضا خارجيا يشك بإصابتهم بداء الليشمانيات الجلدي من مراجعي مستشفى الأمراض الجلدية و الزهرية الجامعي بدمشق.
و فحصت كل عينة من خلال الطرق الثلاث : الفحص المجهري، و الزرع، و تضخيم دنا منشأ الحركة kDNA PCR.
النتائج : كانت حساسية الطرق الثلاث المطبقة على الشكل الغالي : و 9.72 % للطاخة المجهرية، 68.6 % للزرع، 100 % لطريقة kDNA PCR.
الاستنساخ : يبدو أن طريقة kDNA PCR هي الطريفة الأكثر حساسية لتشخيص CL.
و لذلك ينصح بما بشكل خاص في الحالات الني يكون متوقعا فيما سريرا عدوى بالليشمانية مع نتائج زرع أو فحص مجهري سلبية، أو كليهما معا.
Abstract EN
In Syria, cutaneous leishmaniasis (CL) is diagnosed by conventional techniques using microscopic identification and culture.
Due to their high specificity, they are widely spread.
On the other hand, several drawbacks are still limited their application.
Among these are low sensitivity and the length of the test.
Since the past world researchs showed that kDNA PCR is highly sensitive and specific, the objective of our research was to compare the sensitivity of microscopic examination, culture and kDNA PCR based on local samples.
Materials and Methods: samples were obtained from 70 of out patients with suspected CL referred to Damascus Dermatology hospital.
Each sample was examined through the three methods, microscopic examination, culture and kDNA PCR.
Results: Sensitivity of the three applied methods were: 72.9% for microscopic smear, 68.6% for culture, 100% for the kDNA-PCR method.
Conclusion: The kDNA-PCR method appear to be the most sensitive method for diagnosing CL.
It is therefore especially recommended in the cases where leishmania infection is clinicaly suspected and negative results of microscopy or/and culture are obtaind.
