Unilateral exophthalmos is commonly caused by intraorbital tumor.
Orbital varix is a vascular anomaly, accounting for 2 % of orbital tumors.
It generally presents with an intermittent postural exophthalmos.
We present a case report of an orbital varix to discuss diagnostic challenges of this anomaly.
A 33-year-old patient, presented to our department with exophthalmos of the left eye.
The ophthalmologic examination was normal, except a convergence insufficiency.
Orbital imaging tests disclosed a intaorbital vascular lesion with calcification of 4 mm.
Ultrasound Doppler examination showed an anechogenic lesion with an increasing volume in proclive position.
The tumor was diagnosed as orbital varix.
We decided to manage the patient conservatively, in the absence of any complications

L’exophtalmie unilatérale est due le plus souvent à un processus expansif intra orbitaire.
Les varices orbitaires représentant 2 % des tumeurs orbitaires.
Elles se révèlent par une exophtalmie intermittente aggravée par la position proclive.
A travers un cas de varice orbitaire, nous discutons les difficultés diagnostiques de cette entité.
Il s’agit d’une femme, âgée de 33 ans, sans d’antécédents rhinologiques ou traumatiques qui a consulté pour exophtalmie gauche récidivante évoluant depuis une année.
L’examen ophtalmologique était sans particularité, excepté une insuffisance de convergence.
L’examen ORL était sans anomalie.
L’imagerie a mis en évidence une formation orbitaire gauche extra conique accolée à la paroi médiale refoulant le muscle droit interne avec calcification de 4 mm et prise de contraste intense.
L’échographie en position proclive a permis de poser le diagnostic de varice orbitaire.
Nous avons opté pour une attitude conservatrice vue l’absence de complications.
