Abstract EN

Background& Objective: To identefi the Bacterials agents in Meningitis resulting from basal skull Fractures, The antibiotics sensitivity, and The benefit of pneumo 23 vaccine to prevent the occurrence of meningitis in these cases.

Materials & Methods: 46 children who were admitted in Damascus University Children's Hospital with basal skull fracture and meningitis, between 1/1/1997 and 31/12/2008 were included in this study, 28 males and 18 females, the age range was 1-13 years, these children had CSF biochemical and Bacterial Examination, Latex, culture and sensitivity, all patients were given pneumo 23 vaccine.

Results: Meningitis reoccurred in the sigmoid bone fracture only; 43 before the pneumo 23 and 69 after.

From 78 CSF culture, the pathological agent was detected in 46 cases; 36 Pneumococcus, 3 Streptococcus, 2 Klebseilla, 2Moraxella.

1 E coli, 1 Pseudomonas and 1 Staph.

Albicans.

Conclusion: Pneumococcus was the Main cause of bacterial Meningitis associated with basal skull fractures (88%).

Their resistance against antibiotics has been increasing; 48% against penicillin and 10-20% against cephalosporins 3rd and 4th generations.

There was no clear benefit of pneumo 23 vaccine to prevent Bacterial Meningitis in children with basal skull fractures.
