Background & Objective: When the caries become near the pulp in primary molars, the treatment will be the pulpotomy to remove the involved coronal portion of the pulp, preserving the vitality and function of the remaining radicular portion.

Formocresol, Glutar-aldehyde, Ferric sulphate, AMT, Laser application and Electro surgery may used in primary teeth pulpotomy.

Objectives: The aim of this study was to evaluate the clinical and radiographic success of pulpotomy in primary molars using sodium hypochlorite 3% as a one of alternatives of formocresol.

Material and methods: This study performed on 40 primary second molars (20 upper and 20 lower) in healthy children aged 4-9 years, who were treated in the paediatric department at Damascus University in 2008-2009.

The inclusion criteria were: (1) healthy cooperative child, (2) second primary molar with indication for pulpotomy with no sign of mobility, pain or gingival swelling, (3) radiographically; no sign of bone radiolucency or physiological resorption of more than 1/3 of the roots.

After clinical and radiographic assessments and completing of study sheet, regional anaesthetic was applied in the mandible and local anaesthetic was used for upper teeth.

Rubber dam was placed.

Caries was removed using manual excavator and diamond round bur.

The roof of pulp chamber is removed, and then the coronal pulp is amputated using a large excavator or sterile round bur..

Subsequently, cotton pellet dampened with 3% sodium hypochlorite were placed over the pulp tissue for several times until no bleeding achieved.

Any case showed extensive bleeding(more than the normal bleeding time)during pulpotomy was excluded.

The pulp tissue was then covered with zinc oxide -eugenol (ZOE) paste and teeth were finally restored with a stainless-steel crown.

All teeth were followed up clinically and radiographically for 3 posttreatment periods: 3, 6, and 12 months.

Signs of clinical success: (1) absence of pain, (2) absence of mobility, (3) absence of gingival swelling and redness, (4) no fistula formation.

Signs of radiographic success: (1) normal periodontal space surrounding treated teeth, (2) absence of any pathological resorption of the roots.

Results: This study showed after 3 months of follow-up 100% clinical -radiographic success .

At 6 months, the clinical success rate was 100% while the radiographic success rate was 95% .

At 12 months clinical and radiographic success rates were 95% .

There was no statistically significant differences between the success rates in upper and lower teeth ( P > 0.05).

Conclusion: NaOCl 3% can be used successfully as a pulpotomy medicament in primary teeth .
