Background & Objective: The ideal goal of periodontal instrumentation is to effectively remove plaque and calculus without causing root surface damage and various instruments have been designed to achieve this goal.

Root planing aim to create a root surface which is biologically acceptable to surrounding soft tissues and increase root surface roughness.

Aim of study: To evaluate the increasing of root surface roughness produced by hand curettes and piezoelectric ultrasonic instrument with different power settings .

Materials & methods:59 extracted human teeth were divided into 4 groups according the treatment: Gracey curettes, ultrasonic scaler at 20% power, ultrasonic scaler at 50% power and ultrasonic scaler at 100% power, each sample received 15 apical to coronal strokes with a 0-10 degree inclination between the tip and root .

Surface roughness was measured before and after instrumentation.

Statistical analysis was performed at (p<0.05) using SPSS 13 and choosing paired t-test and analysis variance between groups with Bonfferoni post- hoc test.

Results: The roughness average was higher after instrumentation compared before instrumentation.

Significant statistical differences (p<0.05) were observed when comparing the hand group (1.12 ± 0.37) and ultrasonic groups with high power setting (2.35 ± 0.96).

Among ultrasonic groups, the higher power setting produced the higher roughness mean which was significantly higher than the roughness obtained with the low power setting (1.45 ± 0.83).

Conclusion: Ultrasonic scaler groups produced a higher roughness compared to hand group, root surface roughness shows a tendency towards a continuous increase in root surface roughness with increase in power setting.
