Background& Objective: Parkinson disease is a chronic degenerative disease with worldwide distribution.

It affects all races.

Both sexes are nearly equally affected with relative male preponderance.

The familial predisposition in idiopathic Parkinson’s disease was a place of argument for many years.

Materials & Methods: Cross-sectional study of the patients attending the outpatient or the inpatient in the neurological department at the Mouassat Teaching Hospital in Damascus, Syria.

We collected 37 cases who meet diagnostic criteria including at least two of the four cardinal features of Parkinson’s disease: rest tremor, rigidity, bradykinasia, and postural instability.

Adding asymmetry of signs and good response to L-dopa to the criteria.

A detailed history of their controls and closed relatives were obtained.

Our results were compared with the international results.

Results: Although there is increase incidence among the siblings of patients and control group no difference of significance was found (siblings of patients 4.9% while of control group 6.8% (P=0.67) with ( 95% CI=0.08-2.8, OR=0.5).

In our study no children were affected.

On the contrary Parkinson’s disease and dementia involvement were increased among the parents of patients with the illness 18.18% while in the parents of control group was 5.2% which is significant (P=0.04) with (95% CI= 0.06-0.9, OR=0.25).

Paternal transmission was noticeable over the maternal transmission in our study.

There was preponderance to male group (male: female ratio 1.2:1) Conclusion: We conclude that there is a great increase in Parkinson’s disease and dementia in parents of patients with Parkinson’s disease and in siblings.

This increased family distribution indicates genetic factors of dominant somatic trend with age-related low penetrance as well as to associated environmental factors or both
