Introduction: Peripheral facial paralysis (PFP) due to fracture of the temporal bone has increasingly become so frequent.
There is no consensus on explorations or treatment.
Aim: Specify the characteristics of this pathology and propose an appropriate treatment.
Patients and Methods: Retrospective study of 16 cases of posttraumatic PFP.
The degree of PFP was evaluated according to the grading of House and Brackmann.
The paraclinic assessment was based on CT scan of the rocks and EMG of facial nerve.
The treatment was medical (IV corticosteroid) or medicalsurgical.
Results: They were 14 men and 2 women with a mean age of 32 years.
The facial paralysis was immediate after trauma in 13 patients.
Surgical decompression was decided for 5 patients (31%) with average time of 46 days (20 to 90 days).
The rate of improvement in FP was 75% after medical treatment and 60% after surgical treatment.
Discussion: The therapeutic approach and prognosis depends on the severity and delay of the paralysis installation as well as the electrophysiological and evolutionary data.

Introduction: Les paralysies faciales périphériques (PFP) par fracture de l’os temporal sont devenues de plus en plus fréquentes.
Il n’y a pas de consensus quant à la prise en charge.
But : Préciser les caractéristiques de cette pathologie et proposer une conduite à tenir adéquate Patients et Méthodes: Etude rétrospective de 16 cas de PFP post traumatiques.
Le degré de la PF à été évalué selon le Grading de House et Brackmann.
Le bilan paraclinique comportait le scanner des rochers et l’EMG du nerf facial.
Le traitement était médical (corticothérapie IV) ou médico-chirurgical.
Résultats: Il s’agissait de 14 hommes et de 2 femmes d’âge moyen de 32 ans.
La PF était d’installation immédiate dans 13 cas.
La décompression chirurgicale était décidée chez 5 patients (31%) avec un délai moyen de 46 jours (20-90 jours).
Le taux d’amélioration était de 75% après traitement médical et de 60% après traitement chirurgical.
Discussion: Le traitement et le pronostic dépendent aussi bien de la sévérité et du délai de l’installation de la PF que des données électrophysiologiques et évolutives.
