Thoracic Outlet Syndrome is uncommon and the history of it began more than 250 years ago.

The form of thoracic outlet syndrome depends on the structure compressed: neurogenic, venous, or arterial.

Arterial type represents the least common type of thoracic outlet syndrome.

Arterial syndromes are of interest because of their dramatic presentations, and although small in absolute numbers, arterial TOS comprises a large percentage of the serious disabilities resulting from TOS.

Arterial TOS is associated with bony abnormalities in almost all cases, and the cervical rib is the most obvious bony abnormality.

We herein report cases of arterial thoracic outlet syndrome detected in 4 patients with complaint of vascular Symptoms.

First rib and anomalous tissue resection is performed in all patients via a single incision that affords both supraclavicular and infraclavicular exposure.

In conclusion, Arterial Thoracic Outlet Syndrome is a rare syndrome, but it is also the strongest indication for operative intervention.
