Arabic Abstract

المقدمة : مرض السكري من النوع الثاني هو مرض متعدد العوامل و ينشأ عن التفاعل بين عوامل وراثية و بيئية متعددة.

ثم ربط معظم هذه الجينات بخلل في عمل خلايا بيتا، ضعف توازن الجلوكوز و إفراز الأنسولين و معدل انتشاره.

في قطاع غزة معتق كشفت دراسات العلاقة بين العوامل الوراثية على نطاق الجينوم و مرض السكري في مجموعات مكانية مختلفة عن وجود علاقة بين الانماط المختلفة لجينات محددة وخطر الاصابة بمرض السكري.

و كان من بين الانماط الوراثية التي ارتبطت بشكل قوي مع حدوث مرض السكري تلك الموجودة في الجينات التالية : ( IGF2BP2 ( CDKN2A/B re10811661 )( rs4402960 ).

( IGF-2 re680 ).

( HHEX 1111875 الهدف : التحقق في الارتباط بين أنماط اربع جينات وهي ( IGF2BP2 1111875 )، و ( 10811661 CDKN2A/B ) و خطر حدوث مرض السكري من النوع الثاني في قطاع غزة.

الطرق المستخدمة : منهج الدراسة ( مجموعة مرضية - مجموعة ضابطة المجموعة المرضية تحتوي على 100 رجل مريض بداء السكري من النوع الثاني و المجموعة الضابطة تحتوي على 100 رجل من الأصحاء.

تم فحص الطراز الجيني للمجموعتين للأربع جينات باستخدام تقنية ( RFLP-PCR )، بالإضافة إلى قياس مخزون السكر، الأنسولين ( ه- ببتايد )، الكولسترول الدهون الثلاثية الكولسترول عالي الكثافة الكولسترول منخفض الكثافة و تم تحليل البيانات و النتائج باستخدام الاختبارات الإحصائية المناسبة.

النتائج من بين الأشكال المتعددة التي تم اختيارها كانت الارتباطات المهمة واضحة و ذلك بوجود علاقة ذات دلالة إحصائية بين الطراز الجيني ( P-value = 0.0001 ) ( GF22 GG و والاليل T للجين -CDKN2A/B ( P 0.0032 = value ) وحدوث مرض السكر من النوع الثاني الجين 1672 الطراز الجيني AA له تأثير ذات دلالة إحصائية ( 0.036 =P value على زيادة مستوي الكوليسترول منخفض الكثافة في المرضى مقارنة بالأشخاص الذين يحملون الطرار الجيني GG ، من ناحية أخرى الطراز الجيني GG له تأثير ذات دلالة إحصائية ( 0.049=Pvalue ) على زيادة مستوي الكوليسترول عالي الكثافة في المرضي مقارنة بالأشخاص الذين يحملون الطراز الجيني هه - الجين CDKN2AB الطراز الجيني CT له تأثير ذات دلالة إحصائية ( 0.001 =P value ) على خفض مستوي بينايد في المرضى مقارنة بالأشخاص الذين يحملون الطراز الجيني CC.

أظهرت النتائج عدم وجود علاقة ذات دلالة إحصائية لكل من GF2BP2 ( 4402960 TC ) بر HHEX 4G ) ) بين الطراز الجيني والفحوصات المقاسة.

الخلاصة : وجود علاقة ذات دلالة إحصائية لكل من الطرز الجينية للجينات IGF2B2 GG والاليل T للجين CDKN2A/B وخطر الإصابة بالسكر من النوع الثاني في المجموعة المفحوصة الجين IGF2 الطراز الجيني "AA" له تأثير ذات دلالة إحصائية على زيادة مستوى الكوليسترول منخفض الكثافة في المرضى مقارنة بالأشخاص الذين يحملون الطراز الجيني، من ناحية أخرى الطراز الجيني GS له تأثير ذات دلالة إحصائية على زيادة مستوي الكوليسترول عالي الكثافة في المرضي مقارنة بالأشخاص الذين يحملون الطراز الجيني AA - الجين CDKN2A/B الطراز الجيني CT له تأثير ذات دلالة الحصائية على خفض مستوي بينايد في المرضى مقارنة بالأشخاص الذين يحملون الطراز الجيني .CC.

وتوصي الدراسة بتأكيد النتائج التي تم الحصول عليها على عينة أكبر، وفحص الارتباط بين الأشكال المتعددة لجينات اخرى مع مرض السكر من النوع الثاني في سكان قطاع غزة.

English Abstract

Background: Type 2 diabetes mellitus (T2DM) is a multifactorial disease with a strong genetic component interacting with environmental factors.

Most of these genes have been linked to beta-cell dysfunction, impaired glucose homeostasis and insulin secretion.

Its prevalence rate in Gaza Strip is alarming.

Human genome studies revealed many T2DMassociated genetic polymorphisms in various populations.

Among the genes polymorphisms that were strongly associated with diabetes are (IGF2BP2 rs4402960), (IGF-2 rs680), (HHEX rs1111875), and (CDKN2A/B rs10811661).

Objective: To investigate the association between (IGF2BP2 rs4402960), (IGF-2 rs680), (HHEX rs1111875), and (CDKN2A/B rs10811661), genes polymorphisms and T2DM in Gaza Strip.

Methods: In this case-control study, 100 T2DM male patients and 100 control men were examined.

The two groups were genotyped for the four genes polymorphisms using restriction fragment length polymorphism-PCR (RFLP-PCR) technique.

Body mass index (BMI), glycated hemoglobin (HbA1c), insulin (C-peptide), total cholesterol, triglycerides, high density lipoprotein cholesterol (HDL-c), and low density lipoprotein cholesterol (LDLc) were measured for all the study participants.

The relation between the four genes polymorphisms, T2DM and the measured clinical parameters were statistically analyzed using appropriate tests.

Results: Among the tested polymorphisms, significant association were evident between IGF2BP2 "GG" genotype (P-value = 0.0001), and CDKN2A/B T-containing genotypes (Pvalue = 0.0032) and increased risk of T2DM.

The IGF2 "AA" genotype exerts significant effect on LDL-c level in patients (P value= 0.036), LDL-c levels were significantly higher in patients subjects with AA genotype as compared to patients with GG genotype.

Moreover, GG genotype exerted significant effect on HDL-c level in patients (P value=0.049), HDL-c levels were significantly higher in patient subjects harboring the GG genotype compared to patient with AA genotype.

The CDKN2A/B "CT" genotype exerts significant effect on Cpeptide level in patients (P value= 0.001), C-peptide level were significantly lower in patient subjects with CT genotype as compared to patients with CC genotype.

IGF2BP2 (rs4402960 T>C), and HHEX (rs4457053 A>G) genotypes did not reveal significant effects on the tested parameters.

Conclusion: IGF2BP2 "GG" genotype, and CDKN2A/B T allele genotypes are significantly associated with T2DM in the investigated population.

The IGF2 "AA" genotype exerts significant effect on LDL-c level in patients.

Moreover, GG genotype exerted significant effect on HDL-c level in patients.

CDKN2A/B "CT" C-peptide level were significantly lower in patient subjects with CT genotype as compared to patients with CC genotype.

The study recommends confirming the obtained results on a larger sample and examining the association of other genes polymorphisms with T2DM in Palestinian Population.
