An unusual case of NHL in pediatrics was reported.

A 7 year old boy came to Damascus University Children Hospital with all signs of cardiac tamponed 4 days ago.

CXR revealed a remarkable increase of the cardiothoracic index.

Echocardiographic showed pericardial effusion without any anatomical defects, along with a little amount of pleural effusion.

A flowcytometry study of pleural effusion showed T-cells lymphoma immunologic feature.

A cytology study of pleural fluid showed malignant cells and the pathological study of pericardium biopsy revealed an infiltration of small blue round cells in pericardium.

Because it is so rare to have a NHL presentation with only cardiac tamponed manifestation in pediatrics, we report this case is reported and it is recommend that NHL should be in the differential diagnosis of sudden dyspnea in childhood, especially, at school age
