Safety of repeat breast reduction withtransection of the primary pedicle

Objective: To evaluate the safety and applicability of repeat breast reduction using a different pedicle. Methods: Four patients who had breast reduction in the past using inferior pedicle technique had secondary correction using different pedicle.

Only those patients who had transection of the primary pedicle for repeat breast reduction were included. Results: Superiorly based dermoglandular flap was used in the four cases.

The average amount of excised breast tissue on each side was 445 g , and the nipple- areolar complex was transposed superiorly 5 to 5.5 cm.

The follow up period ranged between 1-7 years.

None of the patients had skin or nipple- areolar complex vascular compromise. Conclusion: We think that repeat breast reduction is a safe technique, provided you allow enough time between the primary surgery and the secondary one.

Also, minimal flap dissection and limited nipple-areolar complex transposition are important factors. Key words: Breast Reduction, Superior Pedicle, Vertical Scar
