We report an unusual case of myopericarditis and pericardial effusion with subsequent heart failure, as the initial manifestation of SLE.

The timely recognition and early steroid administration are imperative in SLE-related myopericarditis with cardiomyopathy to prevent the mortality associated with this condition.
