Surgical retrieval of the amplatzer occluder atrial septal defect device from the main pulmonary artery

A 26-year-old male presented with effort intolerance.

Echocardiography showed 27mm secundum atrial septal defect by transthoracic echocardiography or transeosophgeal echocardiograph.

The patient underwent an attempt of trans-catheter closure using a 34mm Amplatzer occluder device.

Balloon sizing of the atrial septal defect was 28mm, with a deficient anterior rim.

An hour after deployment, transthoracic echocardiography showed that the device had embolized to the common pulmonary artery, and lay longitudinally causing partial obstruction of the right ventricular outflow.

Under total cardiopulmonary bypass the device was retrieved through a small pulmonary arteriotomy and the atrial septal defect was closed by a Gore-Tex patch.

The patient had an uneventful postoperative course.
