Objectives : To assess the efficacy and safety of using the method of closed reduction and percutaneous crossed pin fixation in the treatment of severely displaced supracondylar fractures of the humerus in children.

Methods : A total of S3 children with type III supracondylar fractures of the humerus were studied prospectively over a period offour years.

Allfractures were treated by closed reduction and percutaneous crossed pin fixation.

Preoperative findings were recorded.

Clinical and radiological examinations were performed on all patients.

The patients were evaluated by Flynn,s criteria.

Neurovascular complications were also studied.

Results: Of the 53 patients, 48 (90.5 %) had satisfactory functional outcome regarding range of motion.

Ninety four percent of the patients achieved satisfactory cosmetic results.

Two patients developed iatrogenic ulnar nerve palsy and both recovered spontaneously 3 months and 4.5 'months postoperatively.

No residual vascular deficits were noted in any of our patients at the final follow-up.

Conclusion : Pediatric humeral supracondylar fractures are common injuries.

Their management presents a challenge to the treating physician.

Knowledge of these fractures and their potential complications is very important in order to maximize treatment outcome.

This method of closed reduction and percutaneous crossed K-wire fixation is a safe and reliable technique for obtaining and maintaining excellent reduction in this difficult fracture, while preserving vascular integrity.
