Background: Type 2 diabetes is thought to be a polygenic disorder that develops as a result of a complex interaction between multiple genes and environmental factors.

TCF7L2 is one of the most important genes have been associated with T2DM in the Genome-wide association studies.

This research aimed to study the association of rs7903146polymorphism of the gene TCF7L2 with T2DM in a sample of Syrian population.

Methods: This case control studyinvolved 138 individuals: the control group (63 individuals) and the T2DM group (75 individuals).

The TCF7L2 rs7903146polymorphism was genotyped by Restriction Fragment Length Polymorphism (RFLP).

Results: The frequency of the risk allele Tin the T2DM and healthygroups (38.7% vs 30.2%) respectively, and the difference was statistically insignificant (P = 0.139).

The frequency of the TT genotype was significantly higher in the T2DM group, compared to the healthy group (16% vs.

3.2%), and it wassignificantly associated with T2DM, with an OR of 5.586 (95% CI 0.144 - 27.286, P = 0.021).

However, the frequency of CT genotype in the two groups were (45.3% vs.

54%), and the difference was statistically insignificant (P = 0.843).

Conclusions: This research showed that rs7903146polymorphism of the TCF7L2 gene is an important risk factor for type 2 diabetes mellitus in a sample of Syrian population.
