الملخص AR
: تم إثبات الدور الممرض لجرثومة الملوية البوابية (Lp) في ورم الغدد الليمفاوية الناتج عن شراب الشعير LG MALT بشكل جيد.
الهدف: تقييم فائدة استئصال Hp في علاج هذه وLG وتحديد العوامل التنبؤية للاستجابة لهذا العلاج.
الطرق: قمنا بدراسة ذات أثر رجعي مدتها 18 عاما لحالات من MALT.
تم جمع البيانات السريرية والبيولوجية والتنظيرية والنسيجية والعلاجية.
النتائج: قمنا بتجميع 38مريضا بمتوسط عمر 51.8 سنة ونسبة جنس (ذكر / أنثى) 0.9.
أظهر التنظير وجود التهاب معدي زائف في 42.1».
كانت جرثومة HP إيجابية في جميع الحالات.
تم تحقيق تراجع كامل ل ن1 بعد القضاء على Hp في 78.9 .
كانت العوامل المرتبطة بهذا الهدوء هي درجة ON,IS l> ، وغياب متلازمة التهابية ، والمستوى الطبيعي LDH ، والمظهر غير الورمي في التنظير الداخلي ، وغياب سماكة المعدة واعتلال العقد اللمفية في التصوير المقطعي و رتبة IE.
الخلاصة: تؤكد دراستنا أن استئصال جرثومة « هو خط العلاج الأول ل N,IALT LG ويحدد عوامل الاستجابة لهذا العلاج.
الملخص EN
Introduction: The etiopathogenic role of Helicobacter pylori (Hp) in MALT gastric lymphoma (GL) is well demonstrated.Aim: To assess the benefit of Hperadication in the treatment of these GL and to identify predictive factors of response to this treatment.
Methods: This is a 18-yearsfollow up retrospective study of MALT GL cases.Clinico-biological, endoscopic, histological and therapeutic data were collected.Results: We collected 38 patients with a mean age of 51.8 years and a sex ratio (M/F) of 0.9.
Endoscopy showed a pseudo-gastritic form in 42.1%.
The Hp status was positive in all cases.
Complete remission of GL afterHp eradication was achieved in 78.9%.
The factors associated with this remission were the WHO score <1, the absence of an inflammatory syndrome, the normal level of LDH, the non-pseudo-tumoralform on endoscopy, the absence of gastric thickening and lymphadenopathy on CT scan and ≤ IE stage.
Conclusion: Our study confirms that Hp eradication is the first-line therapy for MALT GL and identifies the factors of response to this treatment.

الملخص FRE
Introduction:Le rôle étiopathogénique de Helicobacter pylori (Hp) dans le lymphome gastrique (LG) du MALT est bien démontré.But:Evaluer l’intérêt de l’éradication de Hpdans le traitement de ces LGetidentifier les facteurs prédictifs de réponse à ce traitement.Méthodes:C’est une étude rétrospective,menée sur une période de 18 ans, colligeant des cas de LGdu MALT.
Les données clinicobiologiques, endoscopiques, histologiques et thérapeutiques étaient recueillies.
Résultats:Nous avons colligé 38 patients dont l’âge moyen était de 51,8 ans et le sex-ratio (H/F) de 0,9.
L’endoscopie montrait une forme pseudo-gastritique dans 42,1%.
Le statut Hpétait positif dans tous les cas.
Une rémission complète du LG après éradication de Hpétait obtenue dans 78,9%.
Les facteurs associés à cette rémission étaient le score OMS <1, l’absence d’un syndrome inflammatoire, le taux normal des LDH, l’aspect non pseudo-tumoral à l’endoscopie, l’absence d’épaississement gastrique et d’adénopathie à la TDM et un stade ≤ IE.
Conclusion: Notre étude confirme que l’éradication de Hp est le traitement de 1èreintention du LG du MALT et identifie les facteurs de réponse à ce traitement.
