انقطاع الطمث الوظيفي هو عبارة عن غياب الإباضة المزمن وغالبا ما يرتبط مع الإجهاد الشديد، فقدان الوزن ,ممارسة الرياضة البدنية المفرطة أو مزيج من هذه العوامل المختلفة.
لا يمكن إجراء التشخيص إلا بعد استبعاد الأسباب الأخرى لانقطاع الطمث.
يتم إجراء التقييم عند النساء اللائي تتجاوز فترة الدورة الشهرية 45 يوما بشكل مستمر أو في حالة انقطاع الطمث أكثر من 3 أشهر.
في حالة انقطاع الطمث الأساسي ، يعد تصوير الجهاز التناسلي أمرا ضروريا.
من الناحية العلاجية ، يعتبر الدعم الغذائي أمرا أساسيا كذلك الدعم النفسي ، و يفضل أن يكون ذلك في العلاج السلوكي.
في حالة الفشل ، يوصى بالعلاج باستخدام هرمون الاستروجين عبر الجلد مع البروجستين عن طريق الفم بدلا من موانع الحمل.
في حالة الرغبة في الحمل ، العلاج بمضخة هرمون إفراز الغدد التناسلية ضروري.

Functional hypothalamic amenorrhea is a form of chronic anovulation that is not due to identifiable organic causes.
The diagnosis is made after excluding the anatomic or organic pathology of amenorrhea.
We suggest diagnostic evaluation in adolescents and women whose menstrual cycle interval persistently exceeds 45 days and/or those who present with amenorrhea for 3 months or more.
In case of primary amenorrhea, imaging of the genital tract is essential.
The first therapeutic line is correcting the energy imbalance to improve hypothalamic-pituitary-ovarian axis function and a psychological support, such as cognitive behavior therapy.
In case of failure we use a transdermal E2 therapy with cyclic oral progestin (not oral contraceptives or ethinyl E2).
In patients wishing to conceive, after a complete fertility workup, we use treatment with pulsatile gonadotropin-releasing hormone as a first line.
الملخص FRE
L'aménorrhée hypothalamique fonctionnelle est une forme d'anovulation chronique souvent en relation avec un stress intense, une perte de poids, un exercice excessif ou la combinaison de ces différents facteurs.
Le diagnostic ne peut être porté qu'après exclusion des autres causes d'aménorrhée.
L'évaluation se fait chez des femmes dont l'intervalle entre les règles excède 45 jours de manière persistante ou en cas d'aménorrhée > 3mois.
En cas d'aménorrhée primaire, une imagerie des voies génitales est indispensable.
Sur le plan thérapeutique, une prise en charge nutritionnelle visant à reprendre du poids est indispensable ainsi qu'un soutien psychologique, de préférence en thérapie comportementale.
En cas d'échec, il est recommandé de prescrire un traitement comportant des estrogènes transdermiques associés à des progestatifs oraux plutôt que des contraceptifs oestroprogestatifs.
En cas de désir de grossesse, un traitement par pompe à GnRH est indiqué en première ligne.
