We report the case of a 33-year-old patient, followed in the hematology department for type3 von Willebrand disease, admitted for acute abdominal pain.

The initial examination showed hypogastric sensibility.

No bleeding symptoms were reported, and the hemodynamic status was stable.

Complementary tests confirmed the presence of a large hemoperitoneum complicating a ruptured hemorrhagic ovarian cyst.

Medical treatment with factor VIII concentrates and factor VIIa recombined was initiated, combined with transfusion of RBC and cryoprecipitate.

The decision for surgical treatment was taken in the second instance due to hemodynamic instability and failure of medical treatment.

The operation was performed under general anesthesia, with evidence of a large hemoperitoneum, two bilateral ruptured ovarian cysts; a peritoneal cleansing and a cystectomy were performed, with careful control of the hemostasis and placement of a redon drain in the cul de sac of Douglas.

The patient was awakened in the intensive care unit with a good evolution.

Nous rapportons le cas d'une patiente âgée de 33ans, suivie au service d'hématologie pour maladie de von Willebrand type3, admise pour douleur abdominale aigue.

L'examen initial a retrouvé une sensibilité hypogastrique, aucun symptôme hémorragique n'a été signalé, l'état hémodynamique était stable.

Des examens complémentaires faits en urgence ont confirmé la présence d'un épanchement de grande abondance compliquant un kyste ovarien hémorragique rompu.

Un traitement médical à base de facteur VIII et VII a été instauré, associé à la transfusion de CGR et cryoprécipités.

La décision de traitement chirurgical a été prise en deuxième intension devant l'instabilité hémodynamique et l'échec du traitement médical.

L'intervention s'est déroulée sous anesthésie générale, avec mise en évidence d'un épanchement de grande abondance, deux kystes ovariens bilatéraux rompus ; une toilette péritonéale ainsi qu'une cystectomie ont été réalisées avec contrôle soigneux de l'hémostase et mise en place d'un drain de Redon dans le cul de sac de Douglas.

Le réveil de la patiente était en milieu de réanimation avec une bonne évolution.
