قمنا بدراسة حالة نادرة لجسم غريب حنجري مزمن تم اكتشافه بالصدفة لدى امرأة تبلغ من العمر 49 عاما و التي قامت باستشارة طبية من أجل ضيق التنفس الشهيقي الانتيابي و الذي تطور منذ حوالي 6 أشهر و المرتبط بعسر البلع المرتفع و الذي و ظهر حتى للمأكولات الصلبة.
أظهر التنظير الداخلي مظهرا منتفخا للسان المزمار و الطرجانيات و الطيات الأذنية.
أظهر التصوير المقطعي المحوسب مظهرا منتفخا في لسان المزمار و الطرجانیات، دون وجود آفات مشبوهة.
خضع المريض و لخزعات حنجرية متعددة تنتهي في ورم حبيبي حنجري غريب.
عندما استأنف المريض الاستجواب، لاحظنا فكرة استنشاق لشوكة سمكة في العامين السابقين للأعراض، و التي أهملها المريض.
الملخص EN
This is a rare case of a chronic laryngeal foreign body of incidental discovery in a 49-year-old woman who consults for a paroxysmal inspiratory dyspnea evolving since 6 months associated with a high solid dysphagia.
Endoscopy showed a stubby appearance of the epiglottis, arytenoids and aryepiglottic folds.
CT showed a swollen appearance of the epiglottis and arytenoids with no suspicious lesions.
The patient had multiple laryngeal biopsies concluding in a laryngeal foreign body granuloma.
Re-examination of the patient found the presence of the notion of inhalation of a fish thorn type in the two years preceding the symptomatology, which was neglected by the patient.
الملخص FRE
Nous rapportons le cas rare d'un corps étranger laryngé chronique de découverte fortuite chez une femme de 49 ans qui a consulté pour une dyspnée inspiratoire paroxystique évoluant depuis 6mois associée à une dysphagie haute au solide.
L'endoscopie a montré un aspect boudiné de l'épiglotte, des aryténoïdes et des replis ary-épiglottiques.
La TDM a montré un aspect tuméfié de l'épiglotte et des aryténoïdes sans lésion suspecte.
La patiente a eu des biopsies multiples laryngées concluant à un granulome à corps étranger laryngé.
A la reprise de l'interrogatoire de la patiente, nous avons constaté la notion d'inhalation d'une épine de poisson dans les deux ans précédent la symptomatologie, qui a été négligé par la patiente.
