Chronic renal failure in Gezira state : aetiology, epidemiology and treatment

This study carried out to determine the epidemiological and aetiologica] features of chronic renal failure in Gezira State during 2000.

Hospital records of 56 patients with chronic renal failure admitted during the year 2000 to Gezira Centre for Dialysis and Renal Transplantation, at Wad Medani, were reviewed retrospectively.

The incidence was found to be 14/1,000,000 for the year 2000 with peak age being 40 to 60 years and 2.3:1 male predominance.

While in 34.0% of the patients with CRF no aetiology could be detected.

The following aetiologies were found to be responsible: hypertension in 16.1% of cases, diabetes mellitus in 14.3%, obstructive uropathy in 10.8%), glomerulonephritis in 5.4%, diabetes mellitus and hypertension together in 5.4%, systemic disease in 3.6%, the nephritic syndrome, Alport's syndrome and polycystic kidney disease for 1.8% each.

Last stage renal disease was reached in 5.4%> cases.

Clinical presentation of CRF in this group included gastro-intestinal symptoms, tiredness, and shortness of breath, loin pain, and lower limb swelling and itching.

Sixty-two percent were treated by haemodialysis and peritoneal dialysis, 13%) were put on conservative treatment, and 25% by other treatments.

Outcome recorded a mortality rate of 30.4% for the year 2000.

Complications included hepatitis C (24%) and hepatitis B (22%) Some preventive measures were recommended.
