الملخص EN

Twenty three consecutive Arab out-patients residing in Qatar with irritable bowel syndrome (I.B.S.) were psychologically, socially and biologically tested.

Most patients were referred by Gastroenterologist and met all the diagnostic criteria of I.B.S .

An objective personality test, the Minnesota Multiphasic Personality Inventory (M.M.P.I.) was administered in the Arabic Language.

Eleven patients (48%) showed Hysterionic and depressive features.

Six patient showed depressive features only (26%).

One patient showed features of mild depression and lying.

One patient showed psychopathic features.

One patient showed features of schizoid, depressive and psychopathic personality.

Two patients failed to do the test.

Dexamethasone suppression test (D.S.T.) was applied on 21 patients.

In all subjects the cortisol level was suppressed i.e.

not significantly higher than 50 nano grams per ml.

Social analysis using direct interview with a social worker was administered.

Fourteen patients were subjected to stressful life events, 11 out of them (78,5%), had marital and/or family psychosocial problems.

Five of all patients (22%) their main social problem was being away from family and residing alone in Qatar.

Four of all patients (17%) showed occupational stress and dissatisfaction.

Six patients (26%) showed no stressful life events.

Control : 80 subjects were investigated, 46% showed abdominal symptoms and 59% showed anxiety and tension symptoms.

Treatment modalities included individual and group psychotherapy, family therapy, relaxation training and hypnosis trans – cutaneous nerve and stimulation and tricyclic antidepressants for a short period.

Eight patients 35% showed complete recovery.

Four patients (17%) showed good recovery.

Four patients (17%) moderate recovery.

Three patients showed good response to antidepressants and hypnorelaxation.

Three patients (13%) did not respond to treatment.

To conclude ; there is an evidence of personality profile mainly characterised by depressive and hysterionic traits associated with stressful life events.

There was no evidence of a biological marker of depression (D.S.T.) An electic therapeutic approach is beneficial to the majority of patients inspite of inability for environment manipulation.

1.1) ont subi des epreuves psychologiques sociaux et biologiques.

La plupart des malades sont adresses par des gasteroenterologues et remplissent les criteres diag- nostiques de S.

Un test objectif de personalite, linventaire mul- tiphasique de personality de Minnesota (M.M.P.I.) est d&ivre en langue Arabe.

Onze malades (48%) ont presente un etat depressif et hys- terique.

Six malades (26%) ont presente un etat depressif Unmaladea presente des etats de depression moderee et menterie.

Un malade a presente un etat de psychopathie.

Un malade a presente des etats schizoide.

depressif et de personahte psychopathique.

Deux malades ont manque le repreuve.

L'epreuve de suppression a la dexamethasone est deiivree d 32 malades.

Le taux de cortisol est freine chez tous les malades mais iln 'est significativement superieur 6 50 nanogrammes par ml.

Une analyse socials utilisant un interview direct avec une assistante sociale est faite.

Ouatorze malades sont assujetis aux evenements stressants de la vie.

11 deux (78.5%) ont eu des probiemes psychosociaux maritaux et/ou famillaux.

Cinq de tous les malades (22%) ont un probieme social principal d 'etre loins de leurs families et habitants seuls a Qatar.

Quatre de tous les malades (17%) ont presente des stress professionnel et de mecontenterie.

Six malades (26%) n ont presente aucun evenement stressant de lavie.

Un groupe temoin de 80 malades ont subi les epreuves.

46% ont pre- sente des symptdmes abdominaux et 59% une symptomatologie ten- sionnelle et d'anxiete.

Les modalites therapeutiques consistaient en psychotherapie indi- viduelle et collective, hypnose et stages de relaxation, stimulation per- cutanee du terminus nerveux et la prescription d 'anti-depressants tricyc- liques durant une periode courte.

Huit malades (35%) sont Gompiefo- ment gueris.

Quatre malades (17%) ont eu un bon retablissement.

Quatre malades (17%) ont un retablissement modere.

Trois malades (13%) ont bien repondu A I'hypnorelaxation et A t'administration d’an- tidepressants.

Trois malades (13%) n 'ont pas repondu au traitement.

En conclusion, il ya une evidence d un profit de personalite principale- ment caracterieepardes traits hysterioniques et depresses associes aux evenements stressants de la vie.

II n'y avait aucune evidence de mar- queur biologique de depression (D.

T.).

La majorite des malades dois beneficier dune approche therapeutique elective malgre Tincapacite de manipuler I'environne- ment.
