Background : iron deficiency is one of the important causes of anemia in chronic renal failure (CRF) patients.

Low-molecular weight iron dextrin (CosmoFer) is used to correct anemia.

Thus, the present study evaluates the impact of uremia, hem dialysis session and intravenous iron therapy on pregnancy associated plasma protein–A (PAPP-A) and CD40 ligand (CD40L) levels as inflammatory markers in CRF / insufficiency patients.

Methods : 40 CRF patients undergoing hem dialysis (HD), 40 CRF patients not yet dialyzed and 40 healthy volunteers were enrolled in the study.

All patients were receiving a single IV infusion dose of CosmoFer weekly (100mg).

PAPP-A, CD40L, Fibrinogen, ferritin and high sensitive C-reactive protein were measured.

Results : PAPP-A and CD40L values in uremic patients were significantly higher than control group.

In HD patients, PAPP-A value was significantly increased after either dialysis or therapy by 1hr as compared with values before dialysis or therapy.

CD40L value was statistically decreased after dialysis as compared with predialysis, while after therapy by 1hr CD40L level showed insignificant increase followed by significant decrease after therapy by 48hr as comparing with before therapy.

In non-dialyzed patients, PAPP-A and CD40L showed insignificant increase after therapy as compared with before therapy.

Conclusion : intravenous iron increases PAPP-A and CD40L level in uremic patients and in this way, it is may be harmful.
