الملخص AR
هدف البحث: تقييم تأثيرات السمنة على اختبارات وظائف الكبد و مشعرات شحوم الدم.
طرق البحث: شملت هذه الدراسة 110 من الأصحاء ظاهريا (70 من حالات البدانة و 40 من الحالات طبيعية الوزن) و ذلك في مدينة الموصل في شمال العراق، تراوحت أعمارهم بين 20-50سنة.
تم إجراء اختبارات وظائف الكبد المتضمنة مستوى خميرة ناقلة أمين الألانين ALT, ناقلة أمين الأسبارتات AST, الفوسفاتاز القلوية ALP, و ناقل الغاما غلوتاميل GGT, مع إجراء مقايسات لمستويات البيليروبين الكلي، المباشر و غير المباشر في المصل، قياس مستوى البروتين الكلي، الألبومين و الغلوبيولين و النسبة بينهما.
من جهة أخرى تم قياس مستويات شحوم المصل الصيامية و التي تضمنت مستوى الكولسترول الكلي TC, الشحوم الثلاثية TG, كولسترول البروتين الشحمي عالي الكثافة HDL, كولسترول البروتين الشحمي منخفض الكثافة LDL, و مشعر تصلب الأوعية.
النتائج: أظهرت مقارنة النتائج الملاحظة لدى حالات السمنة مع الحالات طبيعية الوزن وجود زيادة هامة إحصائيا في فعالية خمائر الكبد المصلية (ALT, AST, ALP, GGT) في حالات السمنة (p < 0.001), مع تناقص هام في مستوى الألبومين و النسبة بين الألبومين و الغلوبيولين لدى حالات السمنة (p < 0.05), دون وجود فارق هام في مستوى البروتين الكلي، الغلوبيولين، البيليروبين (الكلي و المباشر و غير المباشر) بين حالات السمنة و حالات الشاهد.
أظهرت مشعرات شحوم المصل (TC, TG, LDL, و مشعر تصلب الأوعية) زيادة هامة إحصائياً في حالات السمنة (p < 0.001), بينما لوحظ انخفاض مستوى HDL في هذه الحالات.
الاستنتاجات: تمثل السمنة عامل خطورة أساسي في زيادة فعالية خمائر الكبد في المصل و وجود تشحم الكبد، كما أنها سبب أساسي في زيادة مشعرات الشحوم في الدم و زيادة الكولسترول الكلي، الشحوم الثلاثية، كولسترول البروتين الشحمي منخفض الكثافة و مشعر تصلب الأوعية و تناقص مستوى كولسترول البروتين الشحمي عالي الكثافة HDL.
تقترح العلاقة الهامة الكائنة بين خمائر الكبد (ALT, AST, GGT) ومشعرات شحوم المصل (TG, TC, HDL) وجود ارتباط هام بين أذية الخلايا الكبدية و مستويات شحوم المصل.
يعتبر إجراء الاختبارات الدورية لوظائف الكبد و مشعرات شحوم المصل من الأمور شديدة الأهمية في التشخيص الباكر و الوقاية من الاختلاطات القلبية الوعائية و الكبدية المرتبطة بالسمنة.
الملخص EN
Objective: To assess the effects of obesity on liver function tests, and assess its effects on the fasting lipid profile.
Methods: This study included 110 (70 obese, 40 normal weight) apparently healthy subjects in Mosul City, Northern of Iraq, aged 20-50 years.
The biochemical parameters measured were liver function tests that included measurement of liver enzymes activity aspartate aminotrasferase (AST), alanine aminotrasferase (ALT), alkaline phosphatase (ALP), gamma-glutamyl trasferase (GGT), measurement of serum total, direct and indirect bilirubin, measurement of serum total protein, albumin, globulin and A/G ratio.
In addition to measurement of fasting serum lipid profile that included total cholesterol (TC), triglycerides (TG), high density lipoprotein-cholesterol (HDL-C), (Non- HDL-C), low density lipoprotein-cholesterol (LDL-C), atherogenic index.
Results: The comparison of obese subjects with normal weight subjects revealed a highly significant increase in serum liver enzymes activity (AST, ALT, ALP, GGT) among obese subjects with p<0.001, and significant decrease in albumin and A/G ratio among obese subjects with p<0.05, while no significant difference observed in total protein, globulin, bilirubin (total, direct and indirect) between cases and controls.
Lipid parameters including (TC, TG, Non-HDL-C, LDL-C and atherogenic index) have also shown highly significant increase in obese subjects with p<0.001, while HDL-C was reduced.
Conclusions: Obesity is a major factor that contributes to the raised serum liver enzymes activity and the presence of fatty liver.
Obesity is a major cause of increasing lipid profile levels represented by increasing total cholesterol, triglycerides, Non-HDL-C, LDL-C, and atherogenic index, and reducing HDL-C level.
The significant relationship between (AST, ALT, GGT with TC, TG, HDL-C) suggests the important relationship between hepatocellular damage and serum lipid profile levels.
Periodic checking of liver function tests and fasting lipid profile is very important in the early diagnosis and the prevention of obesity associatedliver and cardiovascular complications.
