Evaluation of safety and efficacy of the shikani optical stylet tm for orotracheal intubation

BackgrounqyAiirc the 'Shikani Seeing Stylet™' is a new, inexpensive, reusable high resolution endoscope with a malleable stainless-steel sheath which can be inserted through, a tracheal tube allowing intubation to be performed under direct vision.

Though it was FDA-approved in 1996, it is still relatively unknown to most anesthesiologists and has been the subject of few published studies.

The purpose of the present study was to evaluate the safety and efficacy of the Shikani Optical Stylet™ for orotracheal intubation.

Material and Methocfc : this new device was assessed on 40 patients (ASA I-II; age 23-65) scheduled to undergo elective surgery with tracheal intubation Heart rate (HR), non-invasive blood pressure (MBP), oxygen saturation (SpCfe) and end tidal carbon dioxide (ETCO2) were measured at three different times: To (induction of anesthesia), Ti (beginning of intubation procedure), T2 (end of intubation procedure); also the time interval between Ti and T2 was recorded.

Results : all patients were successfully intubated with the device Thirty patients were intubated at the first attempt (T1-T2 mean time = 95 s); three patients were intubated at the first attempt using cricoid pressure (T1-T2 mean time = 10.5 s) ; five patients were intubated at the second attempt (T1-T2 mean time = 33.4 s) ; two patients were intubated at the third attempt (T1-T2 mean time = 525 s).

The HR, MBP, Sp02 andETC02 remained fairly stable.

Conclusion : on the basis of my preliminary experience with 40 patients, the 'Shikani Seeing Stylet™' seems to be a promising device for airway management These initial positive results should be further studied inpatients with difficult airways.
