Open endoscopic pyelo-lithotripsy : an effective procedure for treating multiple renal stones

Aim of the study: To evaluate the outcome of open endoscopic pyelo-lithotripsy as an effective procedure for treating multiple renal stones.

Patients and method: Forty two patients suffering from multiple pelvicaliceal stones were submitted to this study.

Classic open renal exposure was done.

No mobilization of the kidney was needed.

Pelvic stone was removed by using classic maneuver.

Endoscopic intracranial access was obtained through xylotomy incision.

All calicle stones were removed under vision using derma extraction or inset disintegration.

Intraoperative and postoperative complications were recorded.

Results : No Intraoperative bleeding was observed.

Two patients needed nephrolithotomy (4.8 %).

High grade fever occurred in one patient (2.4 %).

Mild hematuria occurred in six patients for two days (14.3 %).

Moderate hematuria occurred in one patient for three days (2.4 %).

No significant fluid absorption occurred during the procedure.

Significant improvement of glomerular filtration rate obtained during follow up (P = .000).

Stone clearance rate was 97.6 %.

Conclusion : this procedure provides under vision nontranscortical stone access with high clearance rate and low complication rate.
