Background : intraventricular rupture of a brain abscess is a natural but uncommon course of an incompletely or untreated brain abscess.

Though the occurrence is not uncommon, the real event is not seen often in daily practice.

Mortality is high in catastrophic presentation Objective : early diagnosis based on CSF study and neuron-imaging improves the outcome.

Magnetic resonance characteristics of brain abscess and its rupture into the ventricular system are highlighted.

Result : spontaneous intraventricular rupture of brain abscess presented as meningitis and was successfully treated with simple external ventricular drainage (EVD) and parenteral antibiotics.

He made good recovery.

Conclusion : A good outcome can be achieved in a potentially dangerous condition like meningitis due to spontaneous intraventricular rupture of brain abscess if the condition is diagnosed early and treated well in time even by simple maneuvers of external ventricular drainage (EVD) and parenteral antibiotics.
