مقدمة : هنالك زيادة في معدل إنتشار السل الحنجلاب و ذلك لزيادة عوامل اختطاره مثل عدوى و متلازمة فيروس نقص المناعة المكتسبه ((aids/hiv.
يمكن للسل الحنجري أن يصيب أي جزء من الحنجرة و يعتمد في تشخيصه على الاشتباه السريري العالي.
الهدف : هو تقييم المظاهر السريرية للسلال حنجري الموضعي أو المصاحب للسل الرئوي.
المرضى و الطرق : تم إجراء هذه الدراسة المقطعية الاستباقية بمستشفى الأذن, الأنف و الحنجرة التعليمي بالخرطوم في السودان في الفترة (سبتمر 2004-نوفمبر 2006) تم جمع 8 حالات للسل الحنجري من مرضى السل الذين لديهم أعراض بالحنجرة أو الذين تم تشخيص السل لديهم بفحص الأنسجة.
النتائج : كان توزيع المرضى 5 ذكر و 3 إناث تتراوح أعمارهم بين 12 و 70 سنة بمتوسط عمري 41 سنة.
أكثر الأعراض شيوعاً هي الصرير, بحة الصوت, و صعوبة البلع بينما كانت أكثر العلامات شيوعاً هي القرح و العقد في معظم أجزاء الحنجرة.
الخلاصة : إن وجود السل الرئوي ليس ضرورياً لتشخيص السل الحنجري و يمكن التعرف على الحالات عبر الكشف الروتيني بتنظير الحنجرة للمرضى الذين يشكون من بحة الصوت لفترة طويلة أو من مرضى السل الذين لديهم أعراض بالحنجرة.
الملخص EN
Background : Prevalence of laryngeal tuberculosis is rising due to the increase in risk factors like HIV / AIDS.
Any part of the larynx can be affected.
Diagnosis depends on high index of suspicion.
Objectives : To evaluate the clinical manifestations due to localized laryngeal tuberculosis and their association with pulmonary TB.
Patients and Methods : This is a prospective study conducted in ENT Hospital-Khartoum, Sudan from September 2004 to November 2006.
All tuberculous patients with laryngeal symptoms and those diagnosed histologically to have laryngeal tuberculosis were included.
Results : Eight patients were studied ; they were five males and three females, with age range between 12-70 years (mean 41years).
Strider, dysphonia and dysphagia were the main complaints.
Ulcers and nodules were the main findings in all parts of the larynx.
Conclusion : Diagnosis of pulmonary tuberculosis is not mandatory for the diagnosis of laryngeal tuberculosis.
Cases are picked up in routine direct laryngoscopy and biopsy for prolonged dysphonia and / or those with known pulmonary tuberculosis who developed laryngeal symptoms.
