Background : It is well recogni /.ed that rccui disease can occur following malignancy in the head neck region.

This is particularly true for basal cc squamous cell carcinoma, whereby recurrence may o many years following initial surgery or other modali We report here on skin malignancy occurring in the s with skull invasion down to the dura mater.

Comp < resection of scalp, craniectomy, dural resection and re struction including a free tissue transfer was analyze Purpose : To evaluate our experience in full thick scalp resection and reconstruction.

Patients and Methods : At the National Cancer I tute, Cairo University, twelve patients with locally vanccd tumors of the scalp invading the calvarium ' treated with wide local excision of the scalp comb with underlying craniectomy and dural resection if nee Reconstruction was completed using fascial graft fo dura, methyl mcthacrylate for the skull, and either i rotational flap or free tissue transfer.

Results : Between 1998 and 2002, twelve pati with locally advanced tumors of the scalp were subje to this surgery.

All patients successfully tolcratcc procedure and completely recovered with minimal i bidity with acceptable cosmetic results.

Conclusion : With the advent of free tissue trai to cover large defects in the skull and the availabilii polymelhyl mcthacrylate (bone cement) for craniopl large defects in the calvarium can be successfully rest and grafted with satisfactory outcome, improved sal rate, and prolonged survival.
