Background : Germ cell aplasia is a type of azoospermia (total absence of spermatozoa in the ejaculate) in which the tubules, which are reduced in diameter, contain only Sertoli cells and no other cells involved in spermatogenesis.

Hormonal disturbances are also involved.

Objective : to assess hormonal and histopathologically aspects of germ cell aplasia among infertile males in Gaza Strip.

Materials and methods : A total of 375 infertile men visiting the Specialized Medical Centers in Gaza City were subjected for testicular biopsy.

Out of them 74 showed germ cell aplasia.

They were interviewed to fill a questionnaire and blood samples were drawn for determination of testosterone, prolactin, luteinizing hormone (LH) and follicular stimulating hormone (FSH).

Forty four fertile men were served as controls.

Results : Germ cell aplasia represented 74 (19.8 %) among azoospermia men in Gaza Strip as indicated by testicular biopsy.

The mean age of controls and patients was 31.2 ± 5.7 and 29.9 ± 5.9 years, respectively.

The controls and patients who had varicocele were 0 (0.0 %) and 10 (13.5 %) compared to 44 (100.0%) and 64 (86.5 %) who had not (c2 corrected = 4.871, p = 0.027).

In addition, 1 (2.3 %) controls and 17 (23.0 %) patients had hormonal problems versus to 43 (97.7 %) and 57 (77.0 %) who had not (c2 corrected = 7.615, p = 0.006).

Testosterone was significantly decreased in patients compared to controls with percentage difference of 66.0 % (5.0 ± 2.2 v 1.7 ± 1.3 ng / ml, p = 0.000).

In contrast, LH and FSH were markedly increased in patients compared to controls showing percentage differences of 103.2 % and 170.1 %, respectively (6.3 ± 3.1 v 12.8 ± 9.7 mIU / ml, p = 0.000 and 7.7 ± 3.9 v 20.8 ± 14.8 mIU / ml, p = 0.000, respectively).

In comparison with normal reference values, the number of controls who had abnormal levels of testosterone, prolactin, LH and FSH were 7 (4.5 %), 2 (15.9 %), 7 (15.9 %) and 13 (29.5 %), respectively compared to patients of 57 (77.0 %), 36 (48.6 %), 44 (59.5 %) and 67 (90.5%), with p = 0.000.

Clinical picture showed that testosterone levels were markedly decreased in hypo gonadotrophic hypogonadism and hyper gonadotrophic hypogonadism compared to the normal reference values (0.5 ± 0.4 and 0.9 ± 0.5 v 2.0-7.0 ng / ml).

LH and FSH were obviously decreased in hypo gonadotrophic hypogonadism (0.6 ± 0.4 and 0.6 ± 0.5 v 2.0 -3.0 and 2.5 – 10.0 mIU / ml, respectively).

In contrast, marked increase in LH and FSH was observed in primary testicular failure (17.3 ± 6.1 and 28.9 ± 6.3) and hyper gonadotrophic hypogonadism (24.5 ± 2.6 and 37.4 ± 6.7) compared to the reference values of 2.0-13.0 and 2.5-10.0 mIU / ml, respectively.

All infertile men with hypo gonadotrophic hypogonadism and hyper gonadotrophic hypogonadism showed abnormal levels of testosterone in comparison with normal reference values.

Abnormal levels of LH and FSH were generally observed in all infertile men with primary, hypo gonadotrophic hypogonadism and hyper gonadotrophic hypogonadism.

Conclusions : Germ cell aplasia represented 19.8 % of azoospermia men.

Testosterone was decreased whereas LH and FSH were increased.

Clinical picture showed that testosterone was markedly decreased in hypo gonadotrophic hypogonadism and hyper gonadotrophic hypogonadism.

In contrast, marked increase in LH and FSH was observed in primary testicular failure and hyper gonadotrophic hypogonadism.
