-The present investigation was designed to study the ultrastructural, histological and physiological effects of the bodybuilder anabolic steroid sustanon 250 mg (a mixture of four testosterone derivatives) in rats.

Twenty eight male albino rats were divided randomly into four groups; the first group represented the control group and received 0.1ml of corn oil.

The other three groups received three doses of sustanon which included 3, 5 and 10mg/kg body weight (b.w.) intramuscularly (i.m.) once a week for four weeks.

Using sustanon caused a non-significant decrease of the body weight gain when compared with control group.

Among the studied organs, the testes showed a significant decrease in their weight when compared with the control group.

The biochemical results showed that sustanon caused a non-significant increase of alkaline phosphatase (ALP) and significant increase of aspartate aminotransferase (AST), alanine aminotransferase (ALT) and total bilirubin compared with the control group.

Significant increase of serum malondialdehyde (MDA), testosterone, total protein and creatinine were recorded especially in the last two sustanon injected groups.

All the previous changes were dose dependent.

Non-significant change in lipid profile was recorded in sustanon injected groups when compared with the control group, while non-significant increase of serum sodium ions and significant increase of potassium ions were reported in the last dose of sustanon injected rats when compared with control animals.

With respect to chloride ions, all sustanon injected groups showed a non-significant change in comparison to the control group.

With the exception of the last dose of injected sustanon which caused a significant increase of total white blood cells (WBCs) count, all sustanon injected groups showed non-significant increase of the hematological parameters which included WBCs count, red blood cells (RBCs) count, platelets (PLTs) count and hemoglobin (Hb) concentration in comparison with the control.

The ultrastructural and histological study included certain organs such as liver, kidney and testis.

The most important histological changes in the liver of sustanon treated rats were the appearance of dead hepatocytes with condensed or fragmented nuclei and infiltration of the inflammatory cells.

The ultrastructural figures showed these dead cells in more detail like swollen mitochondria and fragmented nuclei.

The kidney sections of the sustanon treated groups showed several dose dependent alterations such as dilatation of the kidney tubules, degeneration of the kidney tubule cells and the presence of intertubular inflammatory cells.

The important ultrastructural changes were the swelling of mitochondria and the presence of dark deposited vacuoles in the cytoplasm of the proximal convoluted tubule (PCT) cells.

Regarding to the testes, several changes in the tissues of this organ were detected in the sustanon treated groups which included the disappearance of spermatozoa, degeneration of the germinal epithelium, the appearance of the vacuoles and observing multinucleated giant cells having apoptotic nuclei.

Dose dependent stages were concluded for these changes.

Arrested spermatogenesis due to sustanon injection has been confirmed by sperm counting results and scanning electron microscope (SEM) figures.
